Fit Notes explained.

Why is it called a fit note?

The term ‘fit note’ replaced the old “sick note” in 2010 to focus on what you can do at work, not just what you can’t. The official name is the Statement of Fitness for Work (Med 3). The note either advises that you are not fit for work or that you may be fit for work with certain adjustments.

When do I need a fit note?

Only when you’ve been off work for more than 7 calendar days in a row. For 7 days or fewer, use your employer’s self-certification process.

How long can a fit note be for?

It varies. Initial notes are often for 1–2 weeks. Ongoing conditions may be covered for 2–4 weeks or longer. The clinician will decide what’s appropriate.

Can you backdate a fit note?

Yes, but only if your medical record (or suitable evidence such as a hospital letter) supports the start date. We cannot usually backdate a note if neither we nor a hospital clinician have seen you during your illness.

Do I need to be seen?

Not always. If we have up-to-date information about your condition, we may issue a fit note without an appointment. If we haven’t assessed you recently, we’ll usually contact you to arrange a review.

What if I can work with adjustments?

Fit notes can recommend that you may be fit to work with adjustments such as amended duties, altered hours, or a phased return. Discuss these options directly with your employer.

What if my employer refuses a fit note with suggested adjustments?

Employers decide whether adjustments are possible. We can only comment on your health and functional ability. You may wish to ask your employer for an occupational health assessment for more specific advice. Please note: occupational health services are not provided by the NHS — they are usually arranged privately by employers.

I have been in hospital. Who issues my note?

The hospital should issue the initial note covering your immediate recovery. Your GP can provide follow-up notes if needed

How long will it take to issue?

We aim to process fit note requests within 2–3 working days. At busy times it may take longer — please submit in good time.

Will you email the note to me or my employer?

We usually send fit notes by secure text or email. We send them to you, not directly to your employer.

Can I go back to work before my fit note has expired?

Yes. You can return to work before the end date on your fit note if you feel well enough and your employer agrees. You don’t need a new fit note to say you’re fit for work. Your fit note is not binding — it’s advice to you and your employer about your fitness at the time it was issued.

My employer wants a fit note for me to return to work – can you provide one?

Fit notes are only used to advise when you’re not fit for work or may be fit with adjustments. There isn’t a separate “return to work” note. If your fit note has expired and you feel well enough, you can usually go back without another note. Please direct your employer to the Government website if they require further information.
